Position Overview:
We are seeking a Nurse Practitioner (NP) to join our team in a case management–focused role. This position is ideal for a detail-oriented, organized provider who enjoys coordinating care, managing treatment plans, and collaborating with patients, providers, and external partners—rather than performing frequent hands-on clinical visits.
The NP will oversee patient cases, monitor progress, and ensure each individual receives comprehensive and timely care following injury. This is a unique opportunity to use your clinical expertise in a role that blends medical knowledge with case oversight and communication.
Key Responsibilities:
- Manage and coordinate patient care plans from intake through recovery.
- Serve as the primary clinical liaison among patients, physicians, and other care providers.
- Review and interpret diagnostic results, treatment plans, and progress reports.
- Monitor patient outcomes and ensure adherence to treatment protocols.
- Provide medical insight and recommendations to ensure optimal recovery and compliance.
- Collaborate with attorneys, insurance representatives, and referring providers as needed.
- Maintain accurate and thorough documentation within the EMR.
- Participate in team meetings and contribute to process improvement and patient experience initiatives.
Qualifications:
- Active and unrestricted Nurse Practitioner license in Arizona.
- Minimum of 2 years of experience in pain management, occupational medicine, or case management preferred.
- Strong clinical assessment and decision-making skills.
- Excellent communication, organization, and follow-up abilities.
- Comfortable working in a fast-paced, collaborative, and evolving environment.

Base Pay: $115,000.00 - $160,000.00 per year
